Discover the Healthy Paths for Strong Brains Program!

The Healthy Paths for Strong Brains program provide integrative support services by well-trained and certified Community Health Workers (CHWs) to help ensure families have access to services such as pediatric screenings, resources, and referrals for ages newborn to 3 years-of-age. In addition, various available resources can be utilized to ensure the child’s brain grows and reaches its full potential.

As a child is identified, Healthy Paths for Strong Brains will detect the need for pre-screening based on initial concerns. If concerns are present, additional screening utilizing the Ages and Stages Questionnaire (ASQ) may be necessary. In all cases, a pediatrician with routine well child visits will be established, and families will be provided with a list of various resources throughout the community. Children will be referred to a partnering organization if additional help is needed.
