eForms are electronic forms used to facilitate changes through PeopleSoft. This includes position data, job data, personal data, payroll data and more. These forms allow departments to create electronic forms within PeopleSoft and have them electronically routed through a workflow approval process that ultimately updates the data within PeopleSoft. UT Share Eform Support

To gain access to eForms, your supervisor will need to contact Help Now - Service Portal and submit an IT Support Reqeuest. They can also do this by sending an email to itsupport@uttyler.edu

If you are an employee from the Health Science Center campus and need access to Peoplesoft for the purpose of submitting or approving eforms on UTTyler Main Campus please submit an Access Request Form.

Important Reminders

Eform Deadlines

Time sensitive eForms must meet payroll processing deadlines. Please review Financial Services web page for complete Payroll Schedules and Final Approved by HR deadlines. 

Eform Processing

  • Data is not updated in PeopleSoft until eForm status is complete.
  • Employees cannot begin work until their eForm is complete.
  • Only one eForm per employee/position should be created at a time to avoid errors.
  • Updates regarding eForm status will be left in the comment section of the eForm. Make sure you review it for pending requests, crminal background check information, etc.
  • Allow 10 business days for eForms to be processed, especially during peak times.

Contract Additions & Changes
  • Modifies an existing contract
  • Adds a new contract to an existing active job record
Create Person of Interest
  • Provides email access for pre-employment and University Affiliates
  • Not for currently active employees
New Position Request
  • Creates new position number
Termination
  • Used to terminate an employee
  • Can be used to terminate one or multiple appointments
Additional Pay
  • To pay current full-time, exempt employees a payment for temporary duties performed
  • Prior approval will be required
Funding Change
  • Updates the funding source for the position
Job Attribute Change
  • Extends the appointment for an existing, paid employee
  • Change in pay for an existing employee
Position Attribute Change
  • Changes to position data such as FTE (hours), Reports To, Department Location
  • Reclassify an existing title to a new title
Transfer
  • Transfers an employee from one position to another position within the same department
  • Transfers an employee from one department to another
  • Used for an existing employee and processed by new (receiving) department

 

 

 


Eform What to check before submitting
Appointment
  • Check funding first
  • Search match
  • End dates for part-time employees (remove if not needed)
  • Headcount on position must be “1”
Contract Additions & Changes
  • Appropriate justification
  • Check contract action (add new or existing)
  • Contract start date must match Effective Date
Create Person of Interest
  • Search match
  • end date (cannot exceed 6 mo)
  • Title must be “other affiliated affiliate”
New Position Request
  • Effective date must be before a potental start date if submitting for new hire
  • Head count of “1”
  • All requests must have the Hiring Authorization DocuSign attached
    Only exceptions: part-time faculty & students
Termination
  • Last date worked is last date of reported time
  • Update physical address
  • Update email address to personal email address
  • Attach justification
    Example: Resignation letter
Additional Pay
  • Detailed Justification for the payment will need to be provided
  • Dates must be in current FY
  • Attach justification Example: Supplemental Pay Form

  • Detailed Justification for the change
  • Full Time & Part Time Staff: Attach approval document for pay corrections and changes, and additional duties.
  • Students: If a request to update wages, check that they have been in the role for at least 6 months
  • Attach current student evaluation If additional duties, attach the list of additional duties
