IACUC Forms

This website is under review. Please email Dr. Anna Kurdowska to request the most current forms.

IACUC requires all researchers (including students) to fill out this form when preparing to work with animals. This form in part satisfies IACUC's responsibility to maintain a safe working environment for the researchers as well as the animals. This form should be completed and returned via email prior to the start of your experiments.

Training

All Principal Investigators (PI), Co-Investigators (Co-PI), and students are required to complete safety training before any IACUC submissions are approved. Successful completion of the training is valid for a period of three (3) years.
