To request a DEADLINE EXTENSION, email your NAME and PharmCAS ID (if applicable) to pharmacy@uttyler.edu 

 NEW Applicants to the College of Pharmacy 

 (Applying on Feb 11 or AFTER)

 

 Applicants Completing the ORIGINAL application (Non-PharmCAS application)

(Applying ON/BEFORE Feb 11)

The Ben and Maytee Fisch College of Pharmacy is NOW in PharmCAS.  You may apply to the College between now and March 2nd through PharmCAS , the national pharmacy college application service. 

The application will be available on PharmCAS starting at 5 pm on Feb 11, 2015.

Required Forms

    1. Before completing any forms, please review the Admission Application Instructions.
    • Complete the PharmCAS application and submit required materials to PharmCAS
    1. Complete the Ben and Maytee Fisch College of Pharmacy Supplemental Application.
    2. Mail the $75 Supplemental Application Fee to the College of Pharmacy (see below).

The Supplemental Application will request information required by The University of Texas at Tyler as part of the admission process.. You will also be asked to provide information on your interest in the Ben & Maytee Fisch College of Pharmacy specifically and learning in groups.

Supplemental Application Fee is $75 and may be paid by check or money order to The University of Texas at Tyler. The application fee should be postmarked no later than March 2, 2015 to the mailing address below.

The applicant will provide you with a Reference Release Form.Using the grey button above, complete and submit the Reference Letter Form electronically to the College of Pharmacy. At the end of the form, there is also an option to upload an additional letter of reference, if desired.The reference letter form works best using the browsers, Mozilla Firefox or Google Chrome.Reference letters must be received by March 2, 2015 before 11:59 pm Eastern Standard Time.
